
Medical forms require time to complete (including chart review and attaching of medical records/reports). There will be a charge for form completion as this is not covered by MSP. Please allow for a minimum of 2 weeks for your forms to be completed and book your appointment accordingly with your family physician. Payment must be made prior to collecting the completed form. We do not accept payment from third parties; you will be responsible for the upfront payment and claiming any applicable reimbursement later.
For phone appointments, physicians may call you within 1 hour of your scheduled time, so ensure your phone is readily available and ringer is audible. For in-person visits, we regret that late arrivals may not be accommodated on the same day. Missed in-person or phone appointments may be charged a no-show fee of $50.
The fastest way to book an appointment is online via our Patient Portal. If you would like to switch a phone appointment to in-person, or vice-versa, please give us a call.
To help expedite your check-in and minimize wait times, please check-in to your appointment using the self-service kiosk located at the front of our reception area. You’ll simply need to scan the back of your health services card to begin the check-in process.
Please note, appointments are required for all prescription refills. Please book your appointment at least 2 weeks prior to your medications running out. Pharmacies can help provide emergency refills if needed. Faxed refill requests are not accepted unless previously discussed with your physician.
For safety and allergy reasons, only certified assistance/guide dogs are permitted in the clinic. Please arrange care for pets prior to your visit.
Physicians at our clinic may use an AI scribe to assist with their duties. Consent from patients will be obtained prior to their first use. For questions about our AI scribe, please consult our provider’s FAQ.
We take the privacy and confidentiality of our patients very seriously. Please note, no video, photo or sound recording is permitted on the clinic premises. We also do not offer wifi access. Acting in discordance with this rule may result in termination of your care at the clinic.
Any aggressive or abusive behaviour towards our staff or physicians will not be tolerated in any manner and may result in the immediate termination of your care at the clinic.
